In-Home Day Program Provider

Position Summary: The Day Program Provider is responsible for delivering person-centered, goal-directed services to individuals with intellectual and developmental disabilities in accordance with the Individual Program Plan (IPP), Title 17 regulations, and agency policies. Services focus on skill development, community integration, safety awareness, and social engagement while ensuring the health, safety, and rights of each client.

Respite Provider

Position Summary: The Respite Provider delivers non-medical, in-home, and community-based support to individuals with intellectual and developmental disabilities in order to provide temporary relief to the primary caregiver. Services are provided in accordance with the Individual Program Plan (IPP), Purchase of Service (POS) authorization, Title 17 regulations, and agency policies, while ensuring the health, safety, and rights of the individual served.

Independent Living Skills Instructor

Position Summary: The Independent Living Skills (ILS) Instructor provides individualized, skill-building services to adults with intellectual and developmental disabilities to support increased independence in daily living. Services are delivered in accordance with the Individual Program Plan (IPP), Purchase of Service (POS) authorization, Title 17 regulations, and agency policies. ILS services focus on teaching, modeling, and reinforcing functional life skills and do not include supervision-only or respite services.
